* doc/Makefile.am: Move $(VAR:MOD) expansions inside "if USE_ASCIIDOC".
* doc/Makefile.am: Use proper variable name for text input files.
* doc/Makefile.am: Initialize vars to empty when !USE_ASCIIDOC.
